I, hereby apply to become a member of the CFMEU (attached to the Construction and General Division, NSW Divisional Branch) and to the CFMEU (New South Wales Branch) (CFMEU NSW) (attached to the Construction and General Division). I agree to obey the rules of the CFMEU and CFMEU NSW.
